Phillips County Health Systems currently offering a exciting career opportunity for an experienced Licensed Registered Nurse with excellent leadership and communication skills. A Transitional Care Manager has a diverse role in managing patient care programs and assisting the Chief Nursing Officer as a Nurse Leader.

Part of the job duties include: 

  • Managing our Skilled Swingbed Program, Home Health Program, and Utilization Review.

  • Over seeing Home Health schedule contacts and coordinator. Participate in Home Health assessments and visits as needed.

  • Work with Risk Management Program and Quality Program.

  • Assisting the Chief Nursing Officer in assuming responsibility for taking Nurse Leader and backup RN calls for the nursing department.

  • Addressing and handling personnel issues to promote a productive and supportive work environment, referring those issues that need further action to the Chief Nursing Officer.

  • Monitor patient care quality through the use of data, periodic patient rounding and intermittently working side-by-side with staff to observe and take needed action to enable the department to achieve its quality goals.

  • Assisting in the development of policies/procedures that meet the needs of the department.

  • Leading assigned departmental quality improvement projects. (QA, Chart Reviews, and Audits.)

 

To be eligible for this position you must be a licensed registered nurse, have your PALS, CPR, TNCC, ACLS, ABLS, and NRP certified.
