As a Community Health Care (CHW) you will support and coach at-risk patients to manage their own health and assist them in navigating the health care and social service systems. A CHW works an integrated role in the network of clinics, hospital systems, and community-based agencies. Community Health Worker is a vital communicator that bridges the gap between healthcare providers and people in need of care. In addition, they serve as a link between patients and health or social service agencies.

The CHW assists patients in the community through home visits, attending medical appointments, and other community-based resources and organizing appointments and transportation for the patient. They provide each patient with education, support, and necessary health-related information and make necessary referrals. 

Minimum Requirements:

  • High School diploma or GED required

  • Must have strong PC proficiency and MS Office skills

  • Must be able to manage time effectively in order to complete tasks and meet deadlines

  • Ability to function effectively in an autonomous work environment and as a team

  • Must have good interpersonal skills with the ability to relate to diverse groups of people and people on all levels

  • Ability to prioritize workload and manage multiple tasks with different deadlines

  • Reliable and accessible transportation along with current insurance coverage

  • Experience working with underserved and diverse populations in community settings

  • Experience in navigating health and social service systems

  • Demonstrated ability to support, coach and motivate patients to reach their goals

  • Additional certifications and education in community health are a plus

Transparency in Coverage Rule

Phillips County Health Systems complies with applicable Federal civil rights laws. Phillips County Health Systems does not discriminate, exclude, or treat individuals differently on the basis of race, color, national origin, age, religion, disability, sex, sexual orientation, or gender identity.

The Department of Health and Human Services’, Transparency in Coverage Rule, requires health plans to create a member-facing price comparison tool and post publicly available machine-readable files. These files must be updated monthly and include in-network negotiated payment rates and historical out-of-network charges for covered items and services, including prescription drugs.  The purpose of this Rule is to help you know the cost of your healthcare before receiving the care. 

No one will be denied access to service due to the inability to pay. All applications for sliding fee schedules and discounts will be accepted. Eligibility is determined by family size and income.
